What is Tension Band Wiring(TBW)?
Tension band wiring is a technique used to treat elbow and kneecap fractures. When a bone fractures, tension is created across the bone, which causes the broken pieces to pull apart. This procedure stabilizes the bone by converting those tensile forces into compressive forces, effectively holding the fragments together. In some cases, K-wire fixation is used as an initial step to temporarily align and stabilize the bone before applying tension band wiring.

Preparation for TBW Surgery
Imaging tests, such as X-rays, and physical examinations are performed to evaluate the fracture and properly plan the procedure. Additionally, the patient is asked to refrain from taking any medications that could interfere with the procedure, such as blood thinners, and is instructed to fast for a few hours.
During the procedure
Once in the procedure room, you will be administered general or local anesthesia to reduce the likelihood of experiencing pain during the procedure. A tiny incision is made at the elbow or knee bone, depending on the fracture site, and the bone fragments are then carefully aligned, a procedure called fracture reduction. K wires are sometimes used to temporarily stabilize the bone before the tension band wiring technique that ensures bone stabilization by converting tensile forces into compressive forces. This technique involves passing, twisting, or looping a stainless steel wire through two drill holes made on either side of the fracture. The wire is then tied into knots to secure it, or the bone is further fixed with screws or plates.

The patient may require a hospital stay for a few days. The recovery period may vary from individual to individual; however, the ideal healing period ranges from a few weeks to months.
Post-Op Care for Tension Band Wiring
- Take enough rest and avoid squatting or running in case of a kneecap (patella) fracture.

- Do wear a splint or cast to ensure movement limitation for a few weeks.
