Most cardiac patients fall into the high-risk category for acute kidney injury (AKI). Contrast-induced acute kidney injury (CI-AKI) is a serious complication following coronary angiography (CAG) and PCI, particularly in patients with chronic kidney disease (CKD). The development of CI-AKI prolongs hospitalization, increases financial burdens, and adversely impacts both short-term and long-term mortality rates, especially in those with advanced CKD.

Ultra-low-dose contrast percutaneous coronary intervention (PCI) is a valuable approach for selected complex, high-risk patients with renal failure. One of the primary objectives of ultra-low-contrast PCI is to reduce the risk of developing postprocedural contrast-induced nephropathy (CIN), which predominantly affects patients with baseline renal dysfunction. ULCPCI can be performed electively in cases of advanced chronic kidney disease. Minimizing contrast exposure and potentially using the least nephrotoxic contrast agents are critical considerations during angiography and PCI in patients with pre-existing CKD.
