Intravascular Ultrasound (IVUS) In Percutaneous Coronary Intervention (PCI) Guidance
Angiography provides a two-dimensional view of a three-dimensional structure and does little to delineate the composition of the coronary artery. Imaging-guided percutaneous coronary intervention (PCI) is associated with better clinical outcomes than angiography-guided PCI.
Intravascular ultrasound (IVUS) is a catheter-based coronary imaging technique. It utilizes the emission and subsequent detection of reflected high-frequency (30-60 MHz) sound waves to create high-resolution, cross-sectional images of the coronary artery. IVUS has been the cornerstone of intracoronary imaging for more than two decades.
IVUS reveals how commonly and extensively plain angiography underestimates the true extent of coronary plaque, the characteristics of plaques prone to rupture and cause acute coronary syndromes (lipid-rich, thin-cap atheroma), and the widespread occurrence of vessel remodeling in response to atherosclerosis.
IVUS Advantages:
- The development of IVUS and the recognition of its images are relatively mature.
- IVUS is able to clearly display the intravascular structure and evaluate plaque composition.
- It may optimize and guide the treatment of coronary interventions.
- It can be used to study the mechanism of plaque progression or regression before and after treatment, vascular remodeling, and restenosis after stent implantation.
